U.S. Based Carriers Checklist of Documents
Plateau Forest Products, LLC (PFP) requires new carriers to be qualified in advance in order to do business. Accordingly, please follow the directions outlined below in order to become one of our qualified carriers:
- Completed and Signed W-9 Tax Form
Please provide reason for submitting one or both tax forms. - Valid Insurance Certificate sent directly from insurance agency with the following minimum coverages and requirements:

| Waiver of Subrogation: | Required |
| Auto Liability: | $1,000,000 Minimum Coverage |
| Additional Insured: | Required |
| Waiver of Subrogation: | Required |
| Cargo: | $100,000 Minimum Coverage |
| Cargo Deductible: | $5,000 or less |
| Certificate Holder: | FOREST CITY TRADING GROUP, LLC & ITS SUBSIDIARIES 10250 S.W. GREENBURG RD, #300 PORTLAND, OR 97223 |
| Workers Compensation coverage based on Province requirements: | OR, If exempt, please provide a letter certifying the reason for exemption. |
| Proof of insurance certificate sent directly from insurance agent: | Required |

- Signed Shipper-Carrier Transportation Services Agreement
- Carrier Set-up Information Sheet
